Case Manager Shelter Plus Care

The Position:
St. Patrick Center, a ministry of Catholic Charities, is hiring a housing-focused Case Manager who will provide wrap-around case management services to Shelter Plus Care clients. This case manager will follow a collaborative case management model and a "meet them where they are" framework. This position collaborates with Affinia Healthcare in order to facilitate the provision of health care and supportive services to ensure that goals and objectives are met. The case manager will use best practices to increase client stability by assisting with immediate permanent housing needs, community resources, life skills, crisis intervention, financial stability, and overall well-being. The salary range is between $40,000 and $50,000 a year. The core working hours are Monday through Friday 8:00 am to 4:30 pm.
