Hospital Homeless Service Coordinator

The Position:
St. Patrick Center is hiring a Hospital Homeless Service Coordinator due to our agency expanding and so is our Hospital to Healthy Housing Program. The coordinator will work in a community setting to engage homeless and/or housing insecure patients, identify and secure services for their physical, behavioral and basic needs outside of emergency medical services, and act as advocate and liaison, consulting with community agencies and providers to maintain coordination of care and transition, among other duties. Moreover, the Hospital Homeless Service Coordinator will facilitate person centered, strength based case management services to individual clients in the office, hospital and community settings. This position works Monday-Friday, 8:00 AM-4:30 PM. The salary range is between $35,000-$45,000. Please see full job description for additional details.
