Prevention Case Manager

The Position:
St. Patrick Center, a ministry of Catholic Charities, is hiring a Prevention Case Manager to play a vital role in helping individuals and families at risk of eviction and homelessness find stability and a place to call home. In the position, you will utilize grant resources, work hand-in-hand with clients to uncover their personal support networks, and connect them with a range of wraparound services offered at St. Patrick Center.
