Senior Case Manager - PSH

The Position:
St. Patrick Center, a ministry of Catholic Charities, is hiring a Senior Case Manager for our Permanent Supportive Housing Program. The Senior Case Manager will maintain program capacity and provide crisis support to the Project LIVE team. This position will monitor the overall outcomes of program participants and provide comprehensive continuum of services and individualized case management, among other duties. Moreover, the Senior Case Manager will need to have experience working with the unhoused population and experience conducting home visits.
