Shallow Subsidy Case Manager Part Time

The Position:
St. Patrick Center is hiring a Shallow Subsidy Case Manager to join the Support Services for Veteran Families (SSVF) team. The Case Manager will work a minimum of 20 hours per week, use best practices to assess the housing needs of veterans who are homeless or at risk of becoming homeless, will utilize a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability, will meet with the veteran families to develop an Individual Service Strategy/Treatment Plan of Care, coordinate individualized services for housing stability, and coordinate individualized services for financial stability, among other duties. Moreover, the Shallow Subsidy Case Manager must have an interest in helping veterans reach their full potential and will also be called upon as needed, to serve clients in the counties of Jefferson, Franklin, St. Francois, Washington, Lincoln, Warren, and St. Charles.
