Transition in Place Case Manager

The Position:
St. Patrick Center is hiring a Transition in Place (TIP) Case Manager, part of the Grant Per Diem (GPD) Department, will use evidence-based and peer-reviewed interventions to assist clients in the transitional housing setting. The case manager will meet with clients weekly to build rapport, assist in maintaining natural support in the community, collaborate, track the success of clients' Individual Service Plan, and coordinate individualized services that are aimed at meeting the client where they are. The case manager will use the Housing First and Harm Reduction models and seek to identify and rectify barriers that are debilitating to the clients right to safe and permanent housing. This position works Monday through Friday from 8:00 AM to 4:30 PM. The salary range is between $40,000 and $50,000. Please see full job description for additional details.
